School Health Clinic Program

Rockdale County Public Schools' employs a either a Registered Nurse, Licensed Practical Nurse, or clinic assistant in each school in order to keep students healthy and attending school. Communicable or infectious diseases are not uncommon in school-age children. Communicable means easily spread to others. For this reason, if your child is suspected of having a communicable disease, it is sometimes necessary that we send them home to prevent spread of the disease throughout the school.

Please be aware that a child presenting with a temperature of 100.4 degree or greater, vomiting more than once, having a runny nose with greenish drainage, or having a persistent cough will be sent home.

Please contact the nurse or clinic aide at your child's school should you have any questions or concerns regarding your child's health.

Immunizations Required for Pre-Kindergarten/Kindergarten, 7th Grade, and 11th Grade Students

Georgia Law (20-2-771) requires a valid certificate of immunization (Form #3231) or a notarized statement of religious exemption (Form #2208) for enrollment for all pre-kindergarten/kindergarten, 7th grade students, 11th grade students (who are 16 years of age), students new to Georgia, and students coming from a private school. The certificate of immunization must be completed by the health department or a private physician. A valid Form #3231 must be marked with either "Date of Expiration" or as "Complete for School Attendance." (A certificate marked with a "Date of Expiration" expires on the future date indicated. A current certificate must be submitted within 30 days of expiration.)

7th Grade Immunization Requirements

In Georgia, all children entering 7th grade, and children who are new entrants into a Georgia school in grades 8 through 12, are required to receive one dose of Tdap vaccine and one dose of meningococcal conjugate vaccine to meet immunization requirements.

11th Grade Immunization Requirements

All students entering 11th grade and who are 16 years of age will need proof of a meningococcal booster shot (MCV4), unless their first dose was received on or after their 16th birthday. Students who are not 16 years old do not have to get the vaccine until their 16th birthday.

Scoliosis Screening 
Scoliosis screening is required for all students in the 6th and 8th grades who attend public school in Georgia.  Parents must submit Form 4400 within the first 90 days of school. There are three (3) options:  1) Have your child's physician fill out Form 4400 (below)   - or -
2) Participate in a school scoliosis screening in November  - or -

3) Opt-out of the screening on Form 4400 (below) and return this to your child's school
